Appointment Scheduling
To schedule an appointment:
  1. Click on the link that corresponds with the eye care provider and location that you want.
  2. Enter your name, your phone number, your email address and the last four digits of your social security number into the new appointment scheduling window.
  3. Select the type of appointment and the available time that works for you.
